Immunization and Health Assessment Requirements

North Carolina laws (G.S.130-A-440 and G.S. 130A- 152a) require that all children, regardless of grade, entering a North Carolina PUBLIC school for the first time must submit proof of a current health physical, specifically documented by the healthcare provider on the NC Health Assessment Transmittal Form as well as a current, up to date immunization record within 30 calendar days of enrollment. Both requirements must be met to avoid school exclusion on the 31st day.
